The 2005 Greater Long Island Dental Meeting will be held at the Hilton Huntington Hotel in Melville, New York on Tuesday, April 19th and Wednesday, April 20th. All sessions will be held in the hotel facilities, including full and half-day scientific programs and clinics, with over 140 Exhibits displaying the latest in technology, services and equipment. GLIDM will be hosting our First Annual Golf Outing on Thursday, April 21st.

MEETING REGISTRATION -

Pre-Registration by Mail or Fax:
Discounted advanced registration must be received by mail or by
fax at the GLIDM office no later than April 1, 2005. Look for the Pre-Registration form on the website home page, www.glidm.org.

On-Site Registration:
Registration will be located on the main floor of the hotel. An additional fee of $25 will be added to the registration fee for all on-site registration of scientific sessions (Exhibits Only Pass is free of charge and can be obtained at Registration).

Please note that course registration will be on a space available basis.

MEETING ONLY - Unlimited courses Tuesday afternoon, Wednesday and Wednesday evening (except for CPR, Infection Control and Risk Management, each with a $75 surcharge)

• Non-ADA Members - $175.

GOLF OUTING ONLY - $155
