DENTISTS –
ADA MEMBERS:
A Certificate of Completion form will be distributed in each scientific presentation. The course code will be released at the close of each session. Fill in your name, address, course date, CDE hours, ADA # and/or AGD # along with your signature. The top portion of the certificate should be taken to the rear isle of the Exhibition Hall for validation, retain the bottom portion for record keeping. Only those copies received will be computed and sent electronically to the prospective organizations above, no mailing of forms by the participant is therefore required.
NON-ADA MEMBERS:
Non-ADA Members are to retain the entire copy for record keeping. No submission of forms to
GLIDM are required.
DENTAL HYGIENISTS –
Dental Hygienists will receive credit hours for each course attended. Upon completion, a Certificate of Completion Form will be issued. Retain the entire copy for record keeping. Any questions regarding hygiene credits should be directed to LIDHA table located next to the GLIDM Registration Desk.
