DENTISTS – Certificate of Completion forms (three part form) are available for all scientific presentations. Fill in your name, address, course date, CDE hours, ADA # and/or AGD # along with your signature. The course code will be released at the close of each session. Both the yellow copy (NYSDA) and the pink copy (AGD) should be taken to the rear isle of the Exhibition Hall for validation. Only those copies received will be computed and sent electronically to the prospective organizations above, no mailing of forms by the participant is therefore required. The white copy is to be retained for participants own record keeping. All Non-ADA members are to retain all three copies for their own record keeping.
DENTAL HYGIENISTS – Dental Hygienists who register for scientific sessions at GLIDM will receive credit hours for each course attended. Upon completion of each course, the same three-part form (above) will be issued for the Hygienists own record keeping, no turn in of forms is required. Any questions regarding hygiene credits should be directed to LIDHA whose table is located next to the GLIDM Registration Desk.
